Water Fasting. What It Is, What Happens and What One Measured Fast Really Shows
Almost every water fasting page on the internet is assembled from other water fasting pages. Studies with no faces in them. Before and after photos with no numbers behind them. This one is different in a boring way that turns out to matter. It is written from inside a doctor supervised 90 day water fast that is still running as you read it.
John is 56, he lives in San Diego and he stepped on a bathroom scale at 228.6 pounds on August 1 2026. Same scale every morning, photographed. Same urine ketone strip every morning, photographed. His doctor drew labs the day before he started, redraws them monthly and can shut the whole thing down whenever she decides to. On the morning of day 43 the scale read 180.7. Everything below is built on those mornings and nothing is rounded to make a point.

What water fasting is
A water fast is water and nothing else, for a set stretch of time. No food. No juice, no broth, no protein shake, no cream in the coffee. It is the strictest form of fasting there is, which is exactly why people search for it and exactly why it needs more care than the internet gives it.
What the body does with that is well described. Stored carbohydrate, called glycogen, carries the first day or so. Once it runs down the liver starts converting fat into ketones and those become the main fuel. That switch is the whole mechanism. Everything people talk about on a water fast, the weight, the clear head, the metallic taste, the appetite disappearing, sits downstream of it.
Counting matters more than people expect. This site counts mornings. Day 1 is the first morning without breakfast. The morning of day 4 is 72 hours, the morning of day 8 is one full week and the morning of day 43 is 42 full days. When a page online says seven days and shows a number, it is worth asking which morning that number came from.
What you can drink and what breaks it
Water. In the strict version that is the entire list.
Most supervised long fasts also allow plain electrolytes dissolved in water, meaning sodium, potassium and magnesium. A body on a multi day fast still needs minerals even though it no longer needs calories. John took Celtic salt and electrolytes for the first six weeks on his doctor’s say so. On day 42 he stopped, because a few days of taking more of them left his stomach queasy and he traced the queasiness back to them. His words that morning were that you have to listen to your body. That is one man adjusting his own protocol under supervision, not advice for anyone else.
The other half of this question has a measured answer, which is rare. On day 30, one third of the way through the 90, John planned and ate one small homemade vegetable broth and a quarter of an avocado. Here is what that cost.
- His scale went from 190.2 on day 30 up to 192.0 the next morning. The first gain of the whole fast.
- His ketone strip fell from 16, the darkest square on the bottle, to 4 overnight and then to blank for the next four mornings.
- It took six mornings to get back to the weight he had been standing on before he ate.
A quarter of an avocado does that after a month. Broth, bone broth, a splash of milk and the rest of the gray area are not small print. His own account is in the day 31 note.
What happens on a water fast, hour by hour and then day by day
This is John’s real arc rather than a generic chart. The weights are his mornings and the strip numbers are the Keto Mojo urine strip matched against the chart printed on the bottle, where 0 is a blank pad and 16 is the darkest square it prints.

Day 3 was the worst day of the fast in his own words. Sluggish, low on energy, a little anxious, my body is stressed and I can feel it. Down 8.0 pounds at the 48 hour mark with the strip still showing nothing, which tells you exactly what those 8 pounds were made of.

The strip shows its first color at the 72 hour mark, a 5 on the chart, the same morning his energy came back. A clear 8 on day 5. A dip to 4 on day 6 alongside leg cramps, which is what a hydration swing looks like on a urine test. By day 7 his appetite was gone and his sense of smell had gone sharp.

The 168 hour mark. Down 17.2 pounds and the first 16 on the chart, the darkest square the bottle prints. Physically fine, mentally cranky, which he says shows up around this point on every long fast he has done.

Roughly a pound a morning with the strip pinned at 16 for more than three straight weeks. He goes under 200 on day 20, out of the obese BMI range on day 23 and past four weeks on day 29. Best sleep in years by day 26. This stretch is uneventful and it is where most of the fat actually goes.

One small broth and a quarter of an avocado the day before and the scale goes up for the only time in the fast. The strip collapses to blank for four mornings. The whole week produced 0.9 pounds where every other full week produced at least 6.4.

Day 37 moved 2.9 pounds, his biggest morning since day 3. The strip climbs back through 1.5 and 4 and returns to 16 on day 39, where it has stayed. The seven mornings from day 36 to day 43 gave up 9.5 pounds, the best full week of the fast after the first one.

Week one is 17.2 pounds. No other week in this fast has come close. That gap is not John being unusual, it is what stored carbohydrate does on its way out. Glycogen is held with roughly three grams of water for every gram of fuel. The first days give back water more than fat. The published reviews of prolonged human fasting describe the same steep opening drop settling into a slower steady loss.
How to read any water fast result you see online. Ask how many days. Ask the starting weight. Ask whether the number came from the first week or a later one. A 17 pound week from 228 pounds and a 6 pound week from 197 pounds came from the same man on the same scale. Only one of them is a rate.
The body scan readings from the same scale tell the quieter half. Body fat went from 42.3 percent on day 1 to 29.1 percent on day 43. Muscle share went from 34.5 to 38.1 percent, which does not mean muscle was built. It means fat left faster than everything else. Muscle became a bigger share of a smaller man. Share is not mass. These are consumer scale readings and the harder evidence is blood.
The ketone strip, the only daily proof
Weight can move for a dozen reasons. The strip is the one thing in this fast that says out loud whether the fuel switch actually happened.
- DAYS 1 TO 3Blank
Three mornings, nothing on the pad, while the scale had already given up 8.0 pounds. That combination is the clearest evidence on this whole site that early water fast weight is water.
- DAY 4First trace at 72 hours
The pad shows color for the first time, a 5 on the chart. Same morning his energy came back.
- DAYS 5 TO 7Swinging
An 8. A dip to 4 with leg cramps. An 8 again. Urine strips move with hydration. A single morning is noise.
- DAYS 8 TO 30Pinned at the darkest square
From the 168 hour mark the pad read 16 every single morning for more than three straight weeks.
- DAYS 31 TO 35Knocked flat by one small meal
The broth and the avocado dropped it to 4 the next morning and then to blank for four mornings running.

What a water fast does to blood
Weight is what people search for. Blood is what the fast is actually for. Two full panels from this fast are published.
- Baseline, Labcorp San Diego, July 31 2026. Drawn fasting the morning before day 1. Exactly one marker came back flagged high, a fasting glucose of 106 against a 70 to 99 reference range. Fasting glucose from 100 to 125 is what the American Diabetes Association calls prediabetes. That single number is the reason this project exists. Sodium 140, potassium 4.6, creatinine 1.05, eGFR 83, AST 22 and ALT 35 all landed inside range.
- Month one, at home kit, August 31 2026, day 31. A wider net of 25 markers. The kit's own scorecard read 36 percent optimal, 60 percent average and 4 percent out of range. Total cholesterol 111, HDL 44.1, LDL 48.4, triglycerides 92.5, ApoB 61.3 and high sensitivity CRP 2.02.
- What the two panels can honestly compare. Kidney filtration improved, eGFR 83 to 103.71, with creatinine falling from 1.05 to 0.804. Alkaline phosphatase came down from 120, near the top of the Labcorp range, to 113. Albumin held steady at 4.9 and 4.95.
- The question still open. The home kit does not test fasting glucose. It measured fructosamine instead, which came back at 231. The rematch on the 106 waits for the next clinic draw.

How long people water fast
Most water fasts people actually attempt run from one day to about three. Past that the fast changes category and needs supervision. Every rung below is a morning John woke up to on this fast. Each one has a real number attached.

Two numbers get quoted a lot and both deserve a caveat. Published human trials mostly cover supervised fasts of roughly 5 to 20 days. A 30 day or 90 day fast sits past where most clinics work. And the 382 day fast that turns up in every comment section is a single case report from the 1960s written up after the fact. It is history. It is not a target.
The benefits, sorted honestly
Fasting pages blur three different things together. These are kept apart on purpose.
- Measured. 47.9 pounds gone in 42 full days on one scale. BMI 34.6 to 27.4. Body fat 42.3 to 29.1 percent. A ketone strip that went from blank to the darkest square and held. Kidney filtration improved between two lab panels.
- Reported. Appetite gone by day 7. Joints quiet getting out of bed from around day 9. Best sleep in years by day 26. Waist from about 40 inches to a loose 36 by day 34. Shirts from extra large to a tight large by day 39. These are John's own words about his own body rather than measurements.
- Claimed but not shown here. Autophagy. The science is real and the 2016 Nobel Prize in Physiology or Medicine went to the work behind it. The hour by hour autophagy timelines all over the internet are not measurements, nothing in a kitchen confirms one and this site will not pretend otherwise. John says plainly that giving his cells quiet time is his reason for doing this. That is his reason, not a result.

The risks that actually matter
- Electrolytes. The mineral balance that runs your heart and nerves does not hold itself steady without food. John hit leg cramps on day 6. This is the risk that turns an extended fast from a personal project into something that needs a doctor.
- Heart rhythm. The dangerous version of an electrolyte problem. You do not feel it coming, which is the entire point of blood monitoring.
- Refeeding. Coming off a long fast carelessly is more dangerous than the fast. The longer the fast the slower the way back.
- Dehydration and the opposite. John missed his water one day and real hunger came back for the first time since day 3. Too much plain water with no minerals brings its own trouble.
- Doing it alone. Past a couple of days, a fast without baseline labs and someone with the authority to stop it is a different activity from what is documented here.
Who should never water fast
- Anyone pregnant or breastfeeding.
- Anyone with diabetes or on blood sugar medication.
- Anyone with heart disease, kidney disease or a history of an eating disorder.
- Anyone under 18.
- Anyone underweight.
- Anyone on daily medication that assumes food.
Past that list, John’s own setup is the honest standard. He is 56, his doctor has known him for years, baseline labs were drawn before day 1, he retests monthly and she can end the fast on any day she chooses. That is why he can do what he does. It is not a reason for anyone to copy it without the same scaffolding.
How to come off a water fast
The way back matters more than the fast and this site has one measured lesson about it rather than a theory.
One small vegetable broth and a quarter of an avocado, after 30 days, put 1.8 pounds on the scale overnight, flattened the ketone strip for four mornings and cost six days to undo. That was a planned single serving inside a fast that then continued. A real break, after a long fast, is a slower and more careful process than that. On a fast of any real length it belongs in a doctor’s hands rather than a blog’s.
John has not broken this fast yet. On day 20 he named 160 pounds as the goal. On day 40, at 184.3, he asked out loud at what point he should stop, because he assumes roughly 10 pounds of water comes back in the first weeks afterward. He has not decided. The day 40 note is him thinking it through on camera.
What this fast cannot tell you
- It is one man. One 56 year old starting at 228.6 pounds under medical supervision. A lighter person loses less and a different body holds water differently. Nothing here is a sample size.
- The glucose question is still open. The 106 that started all of this has not been retested at a clinic. Until it is, the biggest claim this fast could make stays unmade.
- It cannot show you autophagy. A scale, a strip and a blood panel are what this site can actually show.
- It says nothing about what happens after. John completed a supervised 83 day fast years ago and regained the weight. He says so himself. Results during a fast and results a year later are two different pages and only one of them has been written.
- It is not medical advice. It is a public record of one supervised fast, published daily, including the parts that went backwards.

Why this page exists. Most water fasting content is written by people who have not done it. This one is written from a fast that is still running, which means it can be wrong tomorrow and will get corrected when it is. That is the trade. You get real numbers instead of a tidy conclusion.

Every full week after it landed between 0.9 and 9.5. The opening drop is mostly water leaving with stored carbohydrate, which is why anyone quoting a first week number as a daily rate is quoting the water. A lighter person loses less than this.
What can you drink on a water fast?
Water. That is the whole list in the strict form. Many supervised protocols also allow plain electrolytes dissolved in water, which is what John did for the first six weeks on his doctor's say so. He stopped on day 42 because they were making his stomach queasy. Coffee, tea, juice, diet drinks and broth are not part of a water only fast. On day 30 he ate one small vegetable broth and a quarter of an avocado and his ketone strip fell from the darkest square to blank within two mornings, which is what breaking a fast looks like measured.
Is water fasting safe?
A day or two is a different question from a week or a month. The risks that matter on a long fast are heart rhythm, electrolyte disturbance and careless refeeding. None of the three announce themselves before they arrive. John is 56, he fasts under a doctor who has known him for years, he had baseline labs drawn before day 1, he retests monthly and she can end the fast at any point. Does water fasting trigger autophagy?
Autophagy is real cellular recycling and the 2016 Nobel Prize in Physiology or Medicine went to the work behind it. The hour by hour autophagy timelines all over the internet are a different thing. Nobody measured autophagy in a kitchen at hour 18 and no home test shows it. What this fast can actually show is a scale, a urine ketone strip and blood panels. John says plainly that cellular quiet time is why he fasts. That is his reason, not a measurement this site is making for him.
What happens to your body on a water fast?
In John's measured version, day 1 and day 2 run on stored carbohydrate with the strip still blank and 4.4 pounds already gone. Day 3 was the worst day of the whole fast in his words, sluggish and stressed. The strip was still blank at 48 hours. The first trace of ketones landed on day 4 along with his energy. Appetite was gone by day 7. The strip pinned at the darkest square on day 8 and the same week his mood turned cranky. From week two onward it became routine, roughly a pound a day, with the metallic taste of ketosis and one electrolyte scare as the only events.
